Participation Waivers/Disclaimers

Please confirm you and your athlete have read and understand the Ohio Department of Health Concussion Information Sheet.(Required)
Please confirm you and your athlete have watched the required video and read the required SCA Information Handout for Lindsay's Law.(Required)
Please confirm you and your athlete have read and understand the Archdiocese's Charter on Youth Athletics' Code of Conduct.(Required)
Please review the medical release and accept this waiver. To the best of my knowledge my child is physically fit and able to participate in athletics and I agree as a parent/guardian to furnish a doctor’s statement to this effect if requested by the Athletic Booster Board. It is understood that SGCS Athletic Boosters do not take responsibility for the physical fitness of players and as parent/guardian, I bear the responsibility for my child’s physical condition. I hereby agree that SGCS Athletic Boosters and its members, coaches, or officers shall not be liable for any injury or loss which my child may sustain while participating in activities of any kind, and I agree to indemnify and hold harmless SGCS Athletic Boosters and its members, coaches, officers, or designates of any kind from any claim whatsoever.(Required)
